Key Responsibilities

• Triaging emergent situations and providing independent application of advanced nursing skills.
• Monitoring acute and chronic medical conditions in collaboration with Primary Care Providers (PCPs).
• Utilizing care plans to deliver clinic and home-based direct patient care.
• Administering treatments, procedures, and medications as prescribed.
• Conducting regular home visits for wound care, lab draws, assessments, and other nursing tasks.

Required Qualifications

• Associates degree in Nursing; Bachelor’s degree preferred.
• Current RN license from an accredited School of Nursing or University.
• Minimum of one year of experience working with elderly, frail, or long-term care populations.
• Valid and current CPR certification (American Heart Association or equivalent).
• Reliable transportation, valid Michigan driver’s license, and proof of insurance.
• Must be fully vaccinated against influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B, or willing to become vaccinated.
• Must be 18 years or older.

Preferred Experience / Skills

• Home care experience preferred.
• Wound care experience preferred.
