Key Responsibilities

  • Manage the active care and coordination of patients within a primary healthcare setting, collaborating with providers and interdisciplinary teams to establish individualized medical goals.
  • Facilitate multi-disciplinary care conferences for high-risk or complex patients to optimize healthcare outcomes.
  • Oversee a panel of patients, including acute, chronic, and preventive care, through continuous review of electronic health records, outreach, and patient education.
  • Serve as a patient advocate, supporting decision-making, independence, and self-care, while addressing patient needs promptly and efficiently.
  • Coordinate referrals within the organization and with outside facilities, tracking progress and outcomes to ensure timely delivery of care.
  • Perform nursing duties including assessment, education, medication administration, and documentation, maintaining safety and infection control standards.
  • Participate in specialty clinic coordination, managing referrals, scheduling, and pre-appointment diagnostics for various clinics.
  • Act as the primary nurse for specialty clinics, assisting with assessments, procedures, and provider support.
  • Manage postpartum care and newborn immunizations for obstetric patients, working closely with primary care providers.
  • Coordinate telehealth visits, acting as nursing staff for video consultations, and manage referral processes for specialized testing such as exercise treadmill tests.

Required Qualifications

  • Active, unrestricted Registered Nurse license in Alaska
  • At least 1 year of case management experience within the last 2 years
  • Proof of current flu vaccine
  • Fingerprint clearance
  • Must have an active Alaska license at the time of submission

Preferred Experience / Skills

  • Basic Life Support (BLS) certification
  • National Case Management certification
  • Experience with electronic health record systems and care coordination in a tribal or rural healthcare setting
