Key Responsibilities

• Provide patient-centered physical therapy assessments, evaluations, and interventions to promote recovery, mobility, pain relief, and strength enhancement.
• Plan, organize, and deliver specialized therapy programs tailored to individual client needs.
• Document client progress, therapeutic plans, and modifications accurately utilizing EPIC charting systems.
• Educate patients and their families on treatment plans, exercises, and home care strategies.
• Follow clinical practice guidelines and incorporate current research into daily treatment routines.
• Offer guidance and supervision within a team, with potential for independent work on days off.

Required Qualifications

• Valid Minnesota Physical Therapist license.
• Graduation from an accredited physical therapy program.
• Proficiency in EPIC charting.
• Ability to perform evaluations and treatments across diverse patient populations, including all ages.

Preferred Experience / Skills

• Previous experience in inpatient and outpatient therapy settings.
• Familiarity with working independently in a clinic environment.
• Knowledge of current research and best practices in physical therapy.

Additional Information

• This position is the sole physical therapist at the facility, providing inpatient and outpatient care across four exam rooms and a 14-bed Med Surg unit.
• Orientation will be supported by a physical therapy assistant.
• All clinicians must badge in/out at the facility and wear designated scrubs (Polo or business casual).
• Candidates must note if they have previously worked for Sanford Health; those with prior Sanford employment must have the end date at least 6 months prior to submission.
