Full-time, day shifts, 10-hour shifts, traveling assignment

Key Responsibilities

• Provide hospice and end-of-life care, including complex symptom management and patient education
• Conduct comprehensive assessments, participate in goals of care discussions with patients, families, and healthcare teams
• Administer medications via various routes (IV, subcutaneous, oral, rectal, etc.) and manage IV equipment
• Support patients with symptom relief techniques, including use of CADD pumps, BiPAP/CPAP, and suction devices
• Participate in patient visits throughout designated geographic areas, including Chittenden and Grand Isle Counties
• Engage in on-call coverage as needed, with minimal call requirements, during regular and weekend shifts

Required Qualifications

• Minimum of 3 years of clinical experience in community hospice care
• Current BLS certification (AHA)
• Valid nursing license in the state of Vermont
• Strong assessment and critical thinking skills specialized in palliative and end-of-life care
• Proficiency in electronic medical records with excellent typing skills
• Ability to work independently and as part of a multidisciplinary team

Preferred Experience / Skills

• Experience working with terminally ill patients, especially in home health or hospice settings
• Knowledge of advanced symptom management techniques and medication administration
• Comfort participating in complex goals of care conversations
• Valid transportation (personal or rental vehicle) with reliable transportation, especially during winter months
