Registered Nurse - Nurse Care Coordinator
Location: Sitka, AK
Schedule / Job Type: Days, full-time (1 shift per week)
Key Responsibilities:
- Manage and coordinate care for patients, collaborating with healthcare providers and interdisciplinary team members to develop individualized treatment plans.
- Facilitate and monitor patient referrals to specialty clinics, ensuring all necessary information is complete and tracking progress to optimize patient outcomes.
- Serve as the primary clinical contact for patient care, proactively addressing delays or obstacles to ensure timely service delivery.
- Perform various nursing duties, including assessment, documentation, patient education, and administration of medications or therapies.
- Maintain compliance with safety, infection control protocols, and annual competencies, ensuring a clean, safe, and well-equipped working environment.
- Build effective team relationships through collaboration and participation in departmental improvement efforts.
- Perform additional duties and special projects as needed to support comprehensive health services.
Required Qualifications:
- Active, unrestricted RN license in the State of Alaska at the time of application.
- High school diploma or GED; Bachelor’s degree in nursing preferred.
- Minimum of 1 year of case management experience.
- BLS certification.
Preferred Experience / Skills:
- Knowledge of nursing process, safety, and infection control principles.
- Strong assessment, communication, and critical thinking skills.
- Ability to work independently and prioritize tasks in a fast-paced environment.
