Key Responsibilities

• Provide specialized home health nursing care to patients in their residences.
• Manage patient caseloads, performing clinical assessments and delivering personalized care plans.
• Ensure compliance with all documentation requirements, including OASIS data collection.
• Collaborate with multidisciplinary teams to coordinate patient services and optimize outcomes.
• Educate patients and families on health management and home care procedures.
• Maintain accurate and timely documentation of all patient interactions and treatments.

Required Qualifications

• Valid Registered Nurse (RN) licensure.
• Experience in home health nursing and case management.
• OASIS experience is strongly preferred.
• Ability to work independently and manage a caseload.
• No requested time off during the contract period.
• Strong clinical assessment skills.

Preferred Experience / Skills

• Prior travel nursing experience in home health.
• Exceptional communication and organizational skills.
• Knowledge of regulatory requirements in home healthcare.
