- Rotating shift, contract, 8-hour shifts
Key Responsibilities
- Provide skilled nursing care to patients in long-term care and skilled nursing facilities
- Administer medications, treatments, and assessments according to care plans
- Collaborate with interdisciplinary teams to ensure quality patient care
- Document patient progress and observations using electronic medical records
- Cover rotating shifts including evenings, with every other weekend and holiday rotations
Required Qualifications
- Valid LVN or LPN license
- Current CPR and IV certification (preferred for LPNs)
- Experience in LTC or SNF settings
- Ability to work rotating shifts and weekends
Preferred Experience / Skills
- Prior experience with PCC electronic medical records
- Experience with medication administration and patient assessments in long-term care
- Knowledge of common LTC/TCU diagnoses and patient care needs
