• Full-time, Day shifts, 1 shift per week, 9 hours per shift

Key Responsibilities

• Provide compassionate, personalized hospice care to patients in their home or care facilities.
• Assess patient needs, manage symptoms, and develop care plans in collaboration with the interdisciplinary team.
• Educate patients and caregivers, fostering confidence and understanding of care goals.
• Lead and coordinate care for an assigned caseload of patients, ensuring quality and continuity of services.
• Demonstrate critical thinking and clinical expertise across a variety of conditions and symptom management.
• Serve as a team leader to support interdisciplinary collaboration and holistic patient care.

Required Qualifications

• Valid RN license.
• Minimum of two years of acute care experience, hospice experience preferred.
• Strong communication and interpersonal skills.
• Ability to work independently with flexibility and professionalism.

Preferred Experience / Skills

• Experience in hospice or palliative care settings.
• Knowledge of symptom management and end-of-life care.
• Ability to adapt to various patient environments including private homes and care facilities.
