• Saint Johnsbury, VT
Schedule / Job Type
• Part-time, 12-hour day shifts, 3 shifts per week
Key Responsibilities
• Provide skilled home health nursing care to patients in their residences
• Assess patient needs, develop and implement care plans
• Monitor and document patient progress and responses to treatment
• Collaborate with healthcare teams to ensure optimal patient outcomes
• Educate patients and families on health management and procedures
Required Qualifications
• Current and active RN license in the state of Vermont
• Experience in home health or community nursing preferred
• Strong assessment and communication skills
• Ability to work independently and manage a patient caseload
Preferred Experience / Skills
• Previous home health nursing experience
• Knowledge of relevant healthcare regulations and safety protocols
