Key Responsibilities

• Provide comprehensive home health nursing care to patients in their residences.
• Assess patient conditions and develop individualized care plans.
• Administer treatments, medications, and monitor patient progress.
• Educate patients and families on health management and safety.
• Collaborate with interdisciplinary teams to ensure quality patient outcomes.
• Document patient care activities accurately and timely.

Required Qualifications

• Active Illinois nursing license or confirmation that application process has been initiated.
• Must understand and comply with OSF’s Drug-Free Campus policy, including producing a negative drug screen.
• Experience working in home health or similar settings is preferred.
• Ability to work independently and manage a caseload effectively.
• Strong communication and interpersonal skills.

Preferred Experience / Skills

• Prior experience in home health nursing.
• Knowledge of community resources and patient education techniques.
• Bilingual skills are a plus.
