Key Responsibilities

• Provide skilled physical therapy services to home health patients in West Portland and surrounding areas.
• Assess patient needs, develop individualized treatment plans, and document progress using electronic documentation systems.
• Collaborate with caregivers and healthcare team members to ensure quality patient care.
• Maintain compliance with all licensing, certification, and facility policies.
• Travel between patient locations within a designated radius, optimizing driving routes to minimize windshield time.

Required Qualifications

• Minimum of 1 year of skilled home health physical therapy experience.
• Current BCLS certification.
• Valid, pending, or active state license as a Physical Therapist in Oregon.
• Own reliable transportation for local travel.

Preferred Experience / Skills

• Experience working with Oasis documentation systems (EPIC, Rover).
• Comfortable working in a home health environment with flexible scheduling.
• Familiarity with mileage reimbursement and complying with travel policies.
