Key Responsibilities

• Evaluate and treat patients using medically prescribed physical therapy programs to improve function and minimize disability.
• Document clinical observations, treatment plans, goals, and discharge summaries accurately.
• Make initial admission and evaluation visits for new patients.
• Supervise Physical Therapy Assistants as needed.
• Notify the Clinical Team Manager of OASIS scores within established timelines.
• Perform other duties aligned with skills and the organization’s mission.

Required Qualifications

• One year of experience in Home Health therapy.
• Current state licensed or eligible for licensure as a Physical Therapist.
• Ability to evaluate and develop treatment plans for diverse patient populations.
• Strong documentation and communication skills.

Preferred Experience / Skills

• Prior experience working in home health settings.
• Ability to manage a caseload independently.
• Compassionate approach with patients and families.
