• Full-time, Days Shift, 4 days per week

Key Responsibilities

• Provide comprehensive end-of-life care, including symptom management and patient education, in a hospice setting.
• Participate in goals of care discussions with patients, families, and the healthcare team.
• Administer medications via IV, subcutaneous, oral, and other routes, ensuring safe and effective pain control.
• Conduct complex symptom assessments and utilize critical thinking to develop individualized care plans.
• Support patients in both home hospice and inpatient settings, rotating between locations as needed.
• Document patient care accurately using the Net Smart electronic medical record system.

Required Qualifications

• Active Registered Nurse (RN) license.
• BLS certification.
• At least 3 years of prior community hospice experience.
• Excellent typing skills for documentation.
• Ability to manage complex symptom control and end-of-life care.
• Strong communication and teaching skills for patient and family education.

Preferred Experience / Skills

• Experience with high-acuity end-of-life care and symptom management.
• Prior experience with portable medical equipment (e.g., IV pumps, BiPap, suction devices).
• Ability to participate in interdisciplinary care planning and goals of care discussions.
