Key Responsibilities

• Provide hospice and home health social work services, including complex symptom management and patient education.
• Conduct comprehensive assessments, develop care plans, and participate in goals of care discussions with patients and their families.
• Document patient interactions accurately using the designated EMR system.
• Cover a geographic service area, including home visits and facility care, with occasional weekend rotations.
• Collaborate with interdisciplinary team members to ensure high-quality end-of-life care.
• Manage cases that may involve advanced symptom control, psychosocial support, and resource coordination.

Required Qualifications

• Master's Degree in Social Work (MSW)
• Minimum of 3-5 years of clinical experience in hospice or home health settings
• Valid experience with EMR systems (preferably EPIC)
• BLS certification
• Ability to work independently and in a team environment

Preferred Experience / Skills

• Previous travel or flexible scheduling experience
• Strong assessment and critical thinking skills for complex symptom management
• Excellent communication and teaching skills for patient and caregiver education
• Experience with end-of-life care planning and comfort management

Additional Details
