Key Responsibilities

  • Effectively complete OASIS documentation in accordance with agency policies, regulations, and timely manner.
  • Assess patient and caregiver needs utilizing the nursing process to develop and communicate individualized, evidence-based care plans.
  • Serve as Case Manager, actively managing caseloads and coordinating visit schedules to meet productivity standards.
  • Implement and evaluate medical plans of care, nursing interventions, and procedures, ensuring safe, effective, and high-quality patient care.
  • Maintain up-to-date skills in IV therapy, wound VAC, complex wound care, and infection control standards.
  • Collaborate with LPNs and other health care professionals to follow the care plan, delegate tasks, and ensure patient needs are met.
  • Document care accurately, including visit notes, forms, and compliance with agency and regulatory standards.
  • Educate patients and caregivers on medications, treatment options, discharge instructions, and reportable conditions.
  • Identify and document interdisciplinary needs like physical therapy or social work, and communicate effectively across disciplines.
  • Manage patient discharge and transfer processes, ensuring all documentation and paperwork are completed per regulations.
  • Coordinate care with physicians, team members, and vendors, supporting holistic and safe patient and family care.
  • Participate in peer review audits, provide and receive professional feedback.
  • Facilitate case conferences with team members to ensure effective communication and updated care strategies.
  • Advocate for patient-centered care that addresses each family’s specific needs.

Required Qualifications

  • Valid Registered Nurse (RN) licensure in Vermont or eligible for license transfer.
  • Proven experience with home health care and nursing documentation, including OASIS.
  • Strong assessment, communication, and critical thinking skills.
  • Knowledge of infection control standards and high-tech nursing skills (IV, wound care).
  • Ability to collaborate effectively within interdisciplinary teams and delegate appropriately.

Preferred Experience / Skills

  • Experience with complex wound care and specialized procedures (wound VAC).
  • Familiarity with Medicare and Medicaid documentation and forms such as HHABN and Notice of Non-Coverage.
  • Prior experience in a home health or community-based nursing setting.
