Key Responsibilities

• Conduct psycho-social assessments through patient interviews to gather relevant information and evaluate the impact of illness on patients and their families.
• Collaborate within an interdisciplinary team to develop comprehensive care plans and coordinate discharge planning, including referrals for community or social services, home care, or necessary equipment.
• Provide therapeutic support, patient education, and medical guidance to individuals and families, including crisis intervention and professional referrals.
• Maintain timely and compliant documentation of assessments, plans, and interventions in electronic health records.
• Participate in team meetings, care conferences, and liaise with other healthcare providers to ensure coordinated patient care.
• Manage a caseload, ensuring all documentation and follow-up meet departmental and regulatory standards.

Required Qualifications

• Alaska Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) license at time of application.
• Valid vaccination for influenza (flu vaccine).
• Ability to perform mental health evaluations, crisis counseling, and documentation with proficiency.
• Knowledge of community resources, care coordination, and multidisciplinary team collaboration.
• Demonstrated understanding of ethical practice, patient confidentiality, and compliance standards.

Preferred Experience / Skills

• 2-4 years of experience in inpatient or outpatient healthcare settings as a social worker.
• Experience working with diverse populations, including individuals experiencing homelessness, addiction, or severe mental illness.
• Proficiency with electronic health records and healthcare technology tools.
• Strong communication, interpersonal, and conflict resolution skills.
