Key Responsibilities
- Provide speech therapy services to patients in a skilled nursing facility setting.
- Develop and implement individualized treatment plans based on patient needs.
- Collaborate with a multidisciplinary team to ensure quality patient care.
- Document patient progress and maintain accurate therapy records.
- Utilize Rehab Optima (Net Health) for documentation and reporting.
Required Qualifications
- Current and active California state license as a Speech Language Pathologist (SLP).
- Over 1 year of experience in a Skilled Nursing Facility (SNF) environment.
- Proficiency with Rehab Optima (Net Health) software.
Preferred Experience / Skills
- Experience working with diverse patient populations in SNF settings.
- Strong communication and teamwork skills.
