Key Responsibilities

• Manage a caseload of 22-28 home health clients, providing comprehensive nursing care.
• Perform 6-7 clinical units daily, including routine visits, admissions, recertifications, and discharges.
• Navigate and document patient care using EMRs such as NursSmart and Oasis with proficiency.
• Conduct thorough assessments and deliver clinical interventions tailored to patient needs.
• Adapt scheduling to accommodate one weekend shift per month, with time off during weekdays.
• Maintain timely, accurate documentation and compliance with healthcare protocols.

Required Qualifications

• Active AHA BLS certification.
• 3-5 years of recent home health nursing experience.
• Strong clinical and assessment skills with the ability to navigate EMRs effectively.
• Prior travel experience in home health settings.
• Must have reliable transportation; personal or rental vehicle recommended.
• Ability to work Monday through Sunday, with bi-weekly orientation sessions.

Preferred Experience / Skills

• Experience with NetSmart EMR system.
• Familiarity with diverse patient populations and complex clinical scenarios.
• Ability to document thoroughly, including all employment gaps over 90 days.
