• Part-time, outpatient clinic setting, no shift specified, 12-hour shifts, 3 days per week

Key Responsibilities

• Assist registered nurses and licensed practitioners in assessing patient needs, prioritizing care, and establishing care goals based on individual patient factors.
• Contribute to the evaluation and adjustment of care plans by observing, recording, and communicating patient responses to interventions.
• Perform patient care and personal cares, ensuring documentation of assessment findings and nursing interventions.
• Collaborate with the multidisciplinary team to implement patient-centered care plans and contribute to discharge planning and patient education.
• Recognize early signs of patient condition changes, report findings appropriately, and execute prescribed nursing and medical interventions.
• Communicate effectively with patients, families, and team members to maintain quality, continuity of care, and patient education.

Required Qualifications

• Valid Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license.
• Ability to perform assessments, provide patient care, and document accurately.
• Strong communication skills and ability to work collaboratively within a team.
• Experience in outpatient or ambulatory care preferred.

Preferred Experience / Skills

• Prior experience working in outpatient clinics or ambulatory care settings.
• Knowledge of discharge planning and patient education best practices.
• Ability to recognize and respond to changes in patient conditions promptly.
