Key Responsibilities

  • Assess patient and caregiver needs using the nursing process to deliver quality, evidence-based care in compliance with regulations.
  • Develop and communicate personalized care plans, ensuring inclusion of patients and caregivers.
  • Manage assigned caseloads efficiently, including scheduling and documentation, in accordance with agency standards.
  • Complete accurate and timely OASIS documentation, staying current on documentation requirements.
  • Implement medical care plans and nursing interventions, maintaining high standards for patient safety and quality.
  • Perform complex skills such as IV therapy, wound care, and infection control measures.
  • Collaborate with LPNs and interdisciplinary team members to ensure comprehensive care delivery.
  • Evaluate the effectiveness of care plans and modify as needed to meet patient needs and ensure high-quality outcomes.
  • Educate patients and caregivers on treatment options, medication management, discharge instructions, and reportable conditions.
  • Document patient care and interactions thoroughly and accurately in accordance with policies and regulations.
  • Coordinate patient discharges, transfers, and communication with healthcare providers and team members.
  • Participate in quality assurance activities, including peer reviews and case conferences, to maintain and improve care quality.
  • Advocate for patient needs and ensure care plans are patient-centered and responsive.
  • Conduct comprehensive assessments during admission and ongoing IDT meetings, developing individualized care plans within required timeframes.
  • Stay updated on advanced nursing skills and infection control standards to provide safe, effective care.
  • Assist with interdisciplinary coordination to ensure seamless patient care and efficient service delivery.

Required Qualifications

  • Valid Registered Nurse (RN) license in the State of Vermont.
  • Proven experience with the OASIS documentation process.
  • Knowledge of home health care regulations, policies, and best practices.
  • Strong assessment, communication, and patient education skills.
  • Ability to perform high-tech skills such as IV insertion, wound care, and wound VAC management.
  • Competency in interdisciplinary collaboration and case management.
  • Ability to handle a caseload efficiently and prioritize patient needs.

Preferred Experience / Skills

  • Prior experience in home health nursing or similar settings.
  • Experience with Medicare/Medicaid documentation and compliance.
  • Familiarity with high-tech nursing interventions and complex wound care.
  • Bilingual skills or additional language proficiency is a plus.
