Key Responsibilities
- Develop and implement individualized care plans in collaboration with physicians, patients, and families.
- Monitor and assess patients’ clinical, functional, and emotional status; adjust care plans accordingly.
- Coordinate patient care activities across interdisciplinary teams and communicate important updates.
- Educate patients and caregivers on self-care techniques, medication management, diet, and related instructions.
- Document evaluations, treatment goals, and progress in patient records.
- Provide ongoing health counseling and support to enhance patient well-being.
Required Qualifications
- Valid RN license in Vermont or New Hampshire, or an unencumbered compact license.
- Graduate of an accredited nursing school.
- Minimum of 2 years of experience in home health nursing.
Preferred Experience / Skills
- Experience working with diverse patient populations in a home health setting.
- Strong communication and interdisciplinary team collaboration skills.
