• Dayshift, Monday through Friday

Key Responsibilities

• Provide psychotherapy and behavioral health services to adult patients aged 55 and older with complex medical, mental health, behavioral, and substance abuse issues
• Conduct biopsychosocial assessments, develop care plans, and document patient encounters using EPIC EMR
• Manage a daily patient load of 50-60+ individuals, including one-on-one care and group counseling sessions
• Collaborate with multidisciplinary teams within long-term care or adult day care settings
• Participate in patient management, potentially joining the facility permanently after the assignment

Required Qualifications

• Valid Washington State license as a Social Worker or Therapist (must be in-hand at signing)
• Minimum of 1 year of experience working with geriatric populations
• BLS certification
• Proficiency in biopsychosocial assessment, documentation, and care planning
• EPIC EMR experience

Preferred Experience / Skills

• Prior experience in long-term care or adult day care environments
• Ability to work independently within scope, with reliable transportation
• Willingness to work a consistent Mon-Fri schedule and join permanently post-assignment
