- Full-time, night shifts, 12-hour shifts, outpatient / long-term care facility
Key Responsibilities
- Provide skilled nursing care to residents in a long-term care or SNF setting.
- Administer medications and treatments accurately and safely.
- Monitor and document resident health status and respond to changes.
- Collaborate with healthcare team to develop and implement care plans.
- Ensure compliance with all regulatory and safety standards.
Required Qualifications
- Valid LVN or LPN license in the state of New Hampshire.
- Minimum of 1-year experience in long-term care or skilled nursing facility preferred.
- Strong clinical skills and knowledge of resident care protocols.
- Excellent communication and documentation skills.
- Ability to work independently and as part of a team.
Preferred Experience / Skills
- Experience with electronic medical records (EMR) systems.
- BLS certification.
- Prior night shift work experience in a healthcare setting.
