Key Responsibilities

• Provide skilled physical therapy services to patients in a skilled nursing facility.
• Evaluate patients' needs and develop personalized treatment plans.
• Implement and document therapy sessions and progress reports.
• Collaborate with healthcare team to ensure optimal patient outcomes.
• Maintain compliance with all licensing, certification, and facility policies.

Required Qualifications

• Valid Arizona state license to practice as a Physical Therapist or an application submitted with an ETA for license approval.
• Current professional licensure or eligibility to practice physical therapy.
• Excellent communication and patient care skills.
• Ability to work effectively in a team environment.

Preferred Experience / Skills

• Previous experience in a skilled nursing or long-term care setting.
• Knowledge of relevant therapy equipment and documentation processes.
