Key Responsibilities
- Provide patient care in a long-term care or skilled nursing facility setting.
- Follow treatment plans and administer medications accurately.
- Monitor residents’ health status and report changes to nursing staff.
- Ensure compliance with facility policies and healthcare regulations.
- Maintain detailed documentation of patient care activities.
Required Qualifications
- Valid LVN or LPN license in the state of Vermont.
- Own reliable transportation.
- Ability to commit to every weekend schedule.
- Excellent communication and patient care skills.
Preferred Experience / Skills
- Previous experience working in LTC or SNF environments.
- Knowledge of medication administration and resident care procedures.
