- Full-time, 8.5-hour shifts, Days, 5 shifts per week
Key Responsibilities
- Provide direct patient care in long-term care or skilled nursing facility settings
- Monitor and assess residents' health status, ensuring compliance with care plans
- Administer medications and treatments as prescribed by physicians
- Collaborate with interdisciplinary team to develop and update care plans
- Document patient progress and maintain accurate medical records
Required Qualifications
- Valid state Registered Nurse (RN) license
- Current CPR certification
- Proven experience in LTC or SNF environments preferred
- Strong assessment and documentation skills
- Ability to work independently and as part of a team
Preferred Experience / Skills
- Prior experience working with elderly or chronic care populations
- Knowledge of state and federal healthcare regulations
