Key Responsibilities

• Provide physical therapy services to patients in a skilled nursing and rehabilitative environment.
• Develop and implement individualized treatment plans for patients recovering from serious illness or injury.
• Assist with diagnostics and therapy tasks using specialized equipment, including stairs, parallel bars, and home care simulations.
• Collaborate with interdisciplinary team to promote patient recovery and well-being.
• Support both short-term rehabilitation and long-term care needs.
• Maintain accurate documentation of patient progress and treatment plans.

Required Qualifications

• Valid state license as a Physical Therapist.
• Proven experience in a skilled nursing or rehabilitative care setting preferred.
• Ability to work independently and as part of a multidisciplinary team.
• Strong interpersonal and communication skills.
• Knowledge of rehabilitation equipment and therapy techniques.

Preferred Experience / Skills

• Experience working with mental health or dementia units is a plus.
• Familiarity with post-hospital recovery protocols.
• Ability to handle a diverse patient population and adapt therapy plans accordingly.
