Schedule / Job Type
- Full-time, day shift, 5 days per week, 8-hour shifts
Key Responsibilities
- Provide physical therapy services to patients in a home health setting within a 60-mile radius of the office
- Perform OASIS assessments and document care in HCHB software
- Complete all charting by 9 a.m. the following morning
- Maintain timely communication with team members and ensure all documentation requirements are met
- Adhere to compliance standards, including travel and on-call responsibilities as discussed during orientation
Required Qualifications
- Valid Physical Therapist license in Washington and Oregon
- Minimum of 1 year of relevant home health experience
- Experience with OASIS assessments in a home health setting (SOC)
- Proficiency in HCHB (Homecare Homebase) software
- Reliable personal transportation and active driver’s license
Preferred Experience / Skills
- Recent home health experience preferred
- Experience with billing modules during orientation
