• Registered Nurse - LTC/SNF

Schedule / Job Type

• Full-time, days shifts, 12-hour shifts, rotating schedule including weekdays and weekends

Key Responsibilities

• Utilize the nursing process and collaborate with interdisciplinary team members to provide patient care.
• Administer prescribed medications and treatments following approved nursing techniques, policies, procedures, standards of care, and state laws.
• Document patient care activities according to standards of practice.
• Develop, initiate, and execute individualized patient care plans in collaboration with physicians and/or hospital policies, including patient and family education.
• Participate in department or unit performance improvement activities, possibly including shared governance.
• Use clinical expertise to support other units/departments in the patient's longitudinal care plan.
• Exercise critical thinking to ensure patient safety and quality of care, escalating issues appropriately.

Required Qualifications

• Valid Wisconsin RN license
• Current CPR certification
• Ability to float between in-patient units or facilities as needed
• Must have current license and meet state requirements for nursing practice

Preferred Experience / Skills

• Previous experience in long-term care or skilled nursing facilities
• Strong understanding of care plans and interdisciplinary collaboration
