Key Responsibilities
• Assist patients with pain management, mobility improvement, and fall risk reduction.
• Support discharge planning to enhance patients' functional independence.
• Document patient evaluations, progress notes, discharges, daily records, insurance communications, and phone conversations.
• Communicate effectively with other healthcare providers, insurance companies, patients, and rehabilitation staff.
• Maintain cleanliness of treatment areas, assist with laundry and bathroom cleaning, and perform additional duties as needed.

Required Qualifications
• Valid license or certification as a Physical Therapist.
• Experience in patient treatment planning and documentation.
• Strong communication skills for interprofessional collaboration.

Preferred Experience / Skills
• Prior experience in outpatient or inpatient rehabilitation settings.
• Knowledge of therapy documentation systems and insurance procedures.
