Key Responsibilities

• Manage the active care of patients, collaborating with medical providers and interdisciplinary team members to establish individualized healthcare goals.
• Facilitate and coordinate patient referrals to specialty clinics, review referral records, and track progress and outcomes, providing updates to providers.
• Serve as the primary clinical contact for patient care and care coordination, proactively addressing delays and obstacles.
• Perform staff nursing duties, including assessment, documentation, education, and administration of medications or therapies.
• Maintain safety, infection control standards, and proper facility and equipment upkeep.
• Participate in team initiatives and special projects to support comprehensive health services.

Required Qualifications

• Current, unrestricted Registered Nursing license in Alaska or another U.S. state (must apply for an Alaska license prior to start if hired).
• Graduate of an accredited nursing program; Bachelor’s degree preferred.
• Basic Life Support (BLS) certification.
• Clinical competency within 3 months of hire and ongoing every three years.
• Knowledge of nursing process, safety, infection control, and patient assessment.
• Excellent communication skills and ability to work independently and as part of a team.

Preferred Experience / Skills

• Experience in care coordination or case management preferred.
• Familiarity with healthcare referral processes and multidisciplinary teamwork.
• Ability to handle fast-paced environments with multi-tasking skills.
