- Days, Part-time (3 shifts per week), 12.5 hours per shift
Key Responsibilities
- Provide high-quality patient care in a Long-Term Care (LTC) or Skilled Nursing Facility (SNF) setting
- Monitor patient health and vitals, administer medications, and document care activities
- Collaborate with healthcare team to develop and implement individualized care plans
- Ensure compliance with healthcare regulations and facility policies
- Educate patients and their families about health management and post-discharge care
Required Qualifications
- Valid RN license in North Carolina
- Proven experience in LTC or SNF environments preferred
- Strong patient assessment and documentation skills
- Excellent communication and teamwork abilities
Preferred Experience / Skills
- Prior experience with elderly or chronically ill patient populations
- Knowledge of state and federal healthcare regulations
- Ability to work independently and manage a high-volume caseload
