Key Responsibilities
- Evaluate patients and develop individualized treatment plans to improve physical or developmental dysfunction, promoting independence in activities of daily living.
- Reassess treatment plans regularly to ensure progress and adjust goals as needed.
- Instruct patients, families, and caregivers on therapy programs and safe activities, providing written instructions for home care.
- Order, adapt, and instruct on the use of adaptive equipment to enhance safety and independence at home.
- Document all care provided, including admissions and daily notes, adhering to professional and departmental standards.
- Communicate effectively with patients, families, physicians, and healthcare team members to coordinate care and discharge planning.
- Supervise therapy assistants and instruct home health aides/CNA staff in therapy follow-through.
- Participate in team meetings, educational sessions, and special projects to support quality care and departmental goals.
Required Qualifications
- Bachelor's degree in Occupational Therapy or related field
- Valid license to practice as an Occupational Therapist in New Mexico
- BLS Certification
- Valid driver's license with current auto insurance
Preferred Experience / Skills
- At least 1 year of supervised clinical experience in an acute or rehabilitation setting
- Home health experience preferred
