Key Responsibilities

• Evaluate patients and develop individualized treatment plans to improve physical or developmental dysfunction, promoting independence in activities of daily living.
• Reassess treatment plans regularly to ensure progress and adjust goals as needed.
• Instruct patients, families, and caregivers on therapy programs and safe activities, providing written instructions for home care.
• Order, adapt, and instruct on the use of adaptive equipment to enhance safety and independence at home.
• Document all care provided, including admissions and daily notes, adhering to professional and departmental standards.
• Communicate effectively with patients, families, physicians, and healthcare team members to coordinate care and discharge planning.
• Supervise therapy assistants and instruct home health aides/CNA staff in therapy follow-through.
• Participate in team meetings, educational sessions, and special projects to support quality care and departmental goals.

Required Qualifications

• Bachelor's degree in Occupational Therapy or related field
• Valid license to practice as an Occupational Therapist in New Mexico
• BLS Certification
• Valid driver's license with current auto insurance

Preferred Experience / Skills

• At least 1 year of supervised clinical experience in an acute or rehabilitation setting
• Home health experience preferred
