Key Responsibilities

• Provide skilled physical therapy services to patients in a skilled nursing facility (SNF).
• Evaluate patient needs, develop treatment plans, and document progress.
• Utilize a variety of therapy equipment and modalities, including aquatic therapy, parallel bars, and standing frames.
• Collaborate with the healthcare team to ensure optimal patient outcomes.
• Adjust therapy sessions based on caseload fluctuations and individual patient needs.
• Maintain accurate and timely patient records in accordance with facility policies.

Required Qualifications

• Valid state license as a Physical Therapist.
• Proven experience in a SNF or long-term care setting preferred.
• Strong knowledge of physical therapy techniques and equipment.
• Excellent communication and patient care skills.

Preferred Experience / Skills

• Experience with aquatic therapy, gait training, and use of therapy equipment such as NuStep and simulated kitchens.
• Ability to adapt treatment plans to diverse patient populations.
