Key Responsibilities
- Perform comprehensive assessments of patients’ needs and coordinate individualized care plans.
- Provide pain and symptom management consistent with the hospice plan of care.
- Educate and support patients and families regarding disease process, self-care techniques, end-of-life issues, and hospice philosophy.
- Collaborate with the interdisciplinary team including physicians, social workers, chaplains, and volunteers.
- Participate in on-call rotation and work approximately 1-3 shifts per week, including weekends and holidays, with response times within 45 minutes.
- Maintain accurate and timely documentation in compliance with regulatory and organizational policies.
Required Qualifications
- Valid Registered Nurse (RN) license in Utah or a compact state.
- Minimum of 3 years of nursing experience, preferably in hospice, home health, or palliative care.
- Strong clinical assessment skills.
- Excellent communication, interpersonal, and organizational skills.
- Valid driver’s license and reliable transportation for home visits; mileage reimbursement at $0.70 per mile.
- Current BLS certification.
- Ability to respond promptly during on-call shifts.
Preferred Experience / Skills
- Hospice, home health, or palliative care experience preferred.
- Knowledge of Hospicesoft or Careficient EMR systems preferred.
