Key Responsibilities

• Perform comprehensive assessments of patients’ needs and coordinate individualized care plans.
• Provide pain and symptom management consistent with the hospice plan of care.
• Educate and support patients and families regarding disease process, self-care techniques, end-of-life issues, and hospice philosophy.
• Collaborate with the interdisciplinary team including physicians, social workers, chaplains, and volunteers.
• Participate in on-call rotation and work approximately 1-3 shifts per week, including weekends and holidays, with response times within 45 minutes.
• Maintain accurate and timely documentation in compliance with regulatory and organizational policies.

Required Qualifications

• Valid Registered Nurse (RN) license in Utah or a compact state.
• Minimum of 3 years of nursing experience, preferably in hospice, home health, or palliative care.
• Strong clinical assessment skills.
• Excellent communication, interpersonal, and organizational skills.
• Valid driver’s license and reliable transportation for home visits; mileage reimbursement at $0.70 per mile.
• Current BLS certification.
• Ability to respond promptly during on-call shifts.

Preferred Experience / Skills

• Hospice, home health, or palliative care experience preferred.
• Knowledge of Hospicesoft or Careficient EMR systems preferred.
