Schedule / Job Type
- Full-time, Mids shift (8:30 AM - 5:00 PM), 5 days per week
Key Responsibilities
- Provide skilled nursing care to residents in long-term care or skilled nursing facilities
- Assist with patient assessments and develop care plans in collaboration with the healthcare team
- Administer medications and treatments according to physicians' orders
- Monitor residents’ health status and document observations accurately
- Ensure compliance with facility policies and state regulations
Required Qualifications
- Valid Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the state of Delaware
- Minimum of 1 year experience in LTC or SNF setting preferred
- Strong understanding of long-term care patient needs and nursing practices
- Ability to work independently and as part of a team
- Good communication and documentation skills
Preferred Experience / Skills
- Experience with electronic health records (EHR) systems
- Compassionate and patient-focused approach to resident care
