Key Responsibilities
• Provide nursing care to residents in long-term care or skilled nursing facilities
• Administer medications and treatments accurately and on schedule
• Monitor patient health and record vital signs and condition changes
• Collaborate with the healthcare team to ensure quality patient care
• Follow facility policies and procedures to ensure compliance and safety
• Document all care provided and maintain detailed records

Required Qualifications
• Valid LVN or LPN license
• Excellent communication and documentation skills
• Ability to work independently and as part of a team
• Basic knowledge of medical terminology and nursing protocols

Preferred Experience / Skills
• Prior experience in LTC or SNF settings
• Knowledge of Medicaid and Medicare documentation requirements
