Schedule / Job Type

• Full-time, 13-week contract, onsite, days (Monday-Friday)

Key Responsibilities

• Provide outpatient physical therapy services, with some visits in assisted living facilities (approximately 20%)
• Attend daily stand-up meetings at 8:30 AM, Monday through Friday
• Manage patient caseload with 6-7 therapy ratios, ensuring quality care within allocated timeframes
• Complete documentation using Wellsky charting software
• Travel within a 50-mile radius from the facility, averaging 50-75 miles weekly
• Ensure all safety protocols are followed, including professional attire and proper footwear

Required Qualifications

• Current licensure as a Physical Therapist with at least 2 years of recent experience
• Recent travel assignment experience in a homecare/homebase setting
• BLS and First Aid certifications
• Ability to commit to full shifts of 12 hours, Monday through Friday
• Must have COVID vaccination and booster(s) in accordance with facility requirements

Preferred Experience / Skills

• Prior outpatient and assisted living facility experience
• Experience with Wellsky or similar charting systems
• Comfortable traveling and managing a mobile caseload within a 50-mile radius
