Key Responsibilities
• Coordinate utilization management activities across hospital departments to ensure effective patient care and resource utilization.
• Conduct assessments of patient needs and facilitate appropriate transitions to next care settings.
• Collaborate with interdisciplinary teams during care rounds, conferences, and planning to optimize patient outcomes.
• Communicate effectively with patients, healthcare providers, community organizations, and payors using various systems and software.
• Participate in cost- and quality-related initiatives, including discharge planning and hospital reimbursement processes.
• Support patients throughout their hospital stay, ensuring seamless care transitions and resource access.
Required Qualifications
• Associate or Bachelor’s degree in Nursing (RN license).
• 3-5 years of clinical experience in medical-surgical nursing, hospital case management, or home care.
• Proven experience in utilization review and discharge planning.
• Valid BLS certification; CCM or ACM certification preferred.
• Active licensure and strong communication skills in verbal, written, and electronic formats.
Preferred Experience / Skills
• Experience with hospital reimbursement methods and quality measures.
• Strong organizational skills and ability to work independently.
• Experience working with community resources for durable medical equipment and transportation arrangements.
