Key Responsibilities

• Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting.
• Administer medications and treatments in accordance with physician orders and facility policies.
• Monitor and document patient status, ensuring accurate and timely record-keeping.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Ensure compliance with all regulatory and safety standards.

Required Qualifications

• Valid LVN or LPN license in the state of South Dakota.
• Proven experience working in LTC or SNF environments.
• Ability to accurately assess patient conditions and respond appropriately.
• Strong communication and teamwork skills.
• Knowledge of nursing best practices and safety protocols.

Preferred Experience / Skills

• Previous experience with rotating shifts.
• Familiarity with Electronic Medical Records (EMR) systems.
