Key Responsibilities
• Provide skilled physical therapy services to patients in a skilled nursing facility (SNF)
• Develop and implement individualized treatment plans to improve patient mobility and function
• Monitor patient progress and adjust treatment plans as needed
• Document patient care and progress according to facility standards
• Collaborate with healthcare team members to ensure comprehensive patient care

Required Qualifications
• Minimum of 1 year of professional SNF experience
• Valid license or certification as a Physical Therapist in the state of Connecticut
• Strong understanding of physical therapy principles and practices
• Excellent communication and patient care skills

Preferred Experience / Skills
• Experience working in a skilled nursing facility environment
• Ability to work independently and as part of a team
• Knowledge of healthcare regulations and documentation requirements
