Key Responsibilities

• Provide physical therapy services to patients in Skilled Nursing Facilities (SNF) and Assisted Living/Independent Living communities.
• Assess patient needs and develop individualized treatment plans.
• Implement therapeutic interventions to improve mobility, strength, and overall functionality.
• Document patient progress and communicate effectively with healthcare teams.
• Ensure compliance with regulatory and safety standards.

Required Qualifications

• Valid state license as a Physical Therapist.
• Proven experience working with SNF and AL/IL populations preferred.
• Excellent assessment and interpersonal skills.
• Ability to work independently and as part of a team.

Preferred Experience / Skills

• Prior experience in a skilled nursing or assisted living setting.
• Strong documentation and communication skills.
