Key Responsibilities

• Assess referred patients to determine their medical condition, functional capabilities, limitations, and potential for rehabilitation.
• Establish and administer individualized treatment plans based on patient capacity and physician guidance.
• Document patient progress, medical records, and treatment outcomes accurately and thoroughly.
• Educate patients and their families on occupational therapy methods and strategies.
• Supervise and collaborate with Certified Occupational Therapy Assistants (COTAs) and rehab aides to ensure effective treatment delivery.
• Contribute to interdisciplinary team efforts to optimize patient care and facility rehabilitation goals.

Required Qualifications

• Graduation from an accredited occupational therapy program.
• Current state license as an Occupational Therapist in Arizona.
• Current CPR certification.
• Minimum of six months of experience in a long-term care or skilled nursing facility preferred.

Preferred Experience / Skills

• Experience providing occupational therapy services in a rehabilitation or skilled nursing environment.
• Knowledge of Minimum Data Set (MDS) procedures and care planning.
• Strong documentation and communication skills.
