Occupational Therapist - SNF/LTC

Schedule / Job Type:
Full-time, PERM, will accept a travel if and only if they have SNF and Nethealth experience

Key Responsibilities:

• Conduct comprehensive assessments of patients’ functional abilities related to daily living activities (ADLs) and instrumental activities of daily living (IADLs).
• Develop and implement individualized treatment plans to improve motor skills, cognitive function, and independence.
• Collaborate with interdisciplinary teams to coordinate patient care and ensure holistic management.
• Document patient progress accurately and in compliance with regulatory standards.
• Educate patients and families on therapeutic strategies and adaptive equipment to support ongoing recovery and independence.

Required Qualifications:

• Valid Occupational Therapist license in the state of Oregon.
• Minimum of 6 months of experience in SNF or LTC settings.
• Strong clinical skills, particularly in geriatrics and long-term care.
• Excellent communication, assessment, and documentation skills.
• Ability to work effectively in a team and adapt in a fast-paced environment.

Preferred Experience / Skills:

• Experience with geriatric populations in skilled nursing or long-term care facilities.
• Knowledge of regulatory requirements for SNF/LTC settings.
