Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting.
• Manage a patient ratio of 1:1, with up to six patient visits per day.
• Document patient care accurately using EPIC/OASIS systems.
• Travel within the local area, with occasional visits up to 50+ miles.
• Follow all facility and clinical protocols, including COVID vaccination requirements.

Required Qualifications

• Minimum of 1 year of skilled home health experience.
• Valid state license as a Physical Therapist.
• BCLS certification.
• Experience with EPIC/OASIS documentation preferred.

Preferred Experience / Skills

• Prior home health therapy experience with familiarity in outpatient documentation systems.
• Reliable transportation for travel between patient locations.
• Ability to work independently and efficiently in a home health environment.
