- Assess patient needs, develop individualized treatment plans, and record clinical progress.
- Collaborate with healthcare team members to ensure optimal patient outcomes.
- Maintain documentation and adhere to compliance standards.
- Support patient recovery and functional independence through evidence-based practices.
Required Qualifications
- Minimum of 1 year of professional SNF experience.
- Valid state license or licensure eligible as a Physical Therapist.
Preferred Experience / Skills
- Experience working with diverse patient populations in a SNF environment.
- Strong communication and team collaboration skills.
