Key Responsibilities

• Provide nursing care to elderly patients in a skilled nursing facility (SNF) or long-term care (LTC) setting.
• Follow patient care plans and document care efficiently using the electronic medical record system.
• Assist with patient assessments, medication administration, and treatment procedures.
• Collaborate with interdisciplinary team members to ensure quality patient outcomes.
• Cover a schedule of every other weekend and holiday shifts.
• Complete a location-specific orientation and up to 3 days of on-the-floor training.

Required Qualifications

• Valid LVN or LPN license.
• Experience working with elderly patients in LTC or SNF settings.
• Ability to adapt to varying patient ratios and diagnoses.
• Proficiency with electronic medical records, preferably PCC.

Preferred Experience / Skills

• Prior experience in a skilled nursing or assisted living environment.
• Knowledge of geriatric care and common diagnoses associated with elderly populations.
• Flexibility to work both 8-hour and 12-hour shift schedules.
