Key Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility setting.
• Develop and implement individualized treatment plans based on patient needs.
• Document patient progress and communicate with healthcare team members.
• Ensure compliance with all regulatory and safety standards.
• Assist in patient education and discharge planning.

Required Qualifications

• Valid Arizona state license for physical therapy (or proof of application and ETA for license approval).
• Ability to work full-time, five days per week.
• Current CPR certification (if mandated by facility standards).
• Excellent communication and patient care skills.

Preferred Experience / Skills

• Previous experience working in a skilled nursing facility.
• Familiarity with providing therapy services in a long-term care setting.
