Key Responsibilities:

• Conduct patient interviews to gather and evaluate relevant information for treatment planning.
• Ensure patient exams, tests, and procedures align with identified needs.
• Identify and communicate variations from expected findings to the charge nurse.
• Develop and deliver patient education, involving family or significant others as appropriate.
• Prioritize patient requests based on urgency and condition.
• Assess and re-assess patient conditions regularly.
• Coordinate with healthcare professionals to provide comprehensive patient care.
• Delegate tasks to team members when appropriate.
• Support case management efforts to ensure patient success and adherence to care standards.
• Collect and synthesize patient data to develop initial treatment plans.
• Coordinate care with primary care providers and outpatient services.
• Participate in multi-disciplinary treatment planning meetings.
• Respond to case management needs, including diagnostics, treatment, and discharge planning.
• Obtain insurance approvals and referrals for services.
• Provide individual and group therapy sessions in accordance with departmental standards.
• Document interventions and patient responses accurately.
• Support supervision of students and interns to ensure performance and documentation standards are met.
• Pursue ongoing professional development to enhance skills and knowledge.

Required Qualifications:

• Bachelor's degree in Psychology, Human Services, or related field; Master's degree preferred.
• Minimum of 1 year of clinical experience in the Human Services field.
• Demonstrated ability to interact effectively with diverse patient populations.
• Good physical and mental health to meet job demands.
• AHA BLS certification required or obtained within 90 days of hire.

Preferred Experience / Skills:

• Experience in mental health treatment and case management.
• Familiarity with multidisciplinary team collaboration.
• Knowledge of healthcare documentation policies.

Hours Per Shift: 8 hours
