• Full-time, day shifts, 8-hour shifts, 40 hours per week

• Provide physical therapy services to patients following established treatment plans.
• Complete documentation and communicate directly with the facility prior to start.
• Coordinate patient care and ensure compliance with licensing and credentialing requirements.
• Maintain active licensure and equipment needed for therapy sessions.
• Participate in onboarding procedures including parking and dress code compliance.

• Active physical therapy license in the state of Arizona.
• Experience in setting up therapy treatments and documenting patient progress.
• Ability to complete required documentation accurately and timely.
• Availability to work full-time days, Monday through Friday.
• Equipment provided during interview.

• Previous experience working in outpatient or clinical settings.
• Familiarity with credentialing processes and documentation requirements.
